Vehicle certification forms

The Driver & Vehicle Certification Unit (DVCU) performs regulatory functions for vehicle compliance, vehicle inspectors and inspecting organisations, and also investigates vehicle compliance complaints from the public or the motor trade. The majority of these functions require submission of an application form. Please use the correct form and make sure you have the authority to make the application.

Transport Services Delivery Agents: Please note that these forms may differ from the forms you currently have in your Vehicle inspection requirements manuals (VIRMs), including the VIRM: Entry certification, version 2. Your VIRMs will be updated through later amendments. In the meantime either version is acceptable.
